This National Consumer Center scam that you encountered aims to get peoples' information and trick them into believing that they have won something valuable. It is good that you didn't reveal your information, so you may possibly avoid additional issues regarding the scam and advertising material.
However, the involvement with Amarktflow and this scam indicate that your device may be infected by an adware program that can, unfortunately, affect more parts of the computer and interfere with the system settings, functions. You need to scan the machine with the anti-malware tool and see is there any PUPs running behind your back that trigger those redirects and malicious survey forms.
Try to avoid clicking on any advertising material and check the system for infections with automatic programs, so all traces can get terminated and cleaned.

If you need security software that would detect and remove adware from your mobile phone, we would recommend Malwarebytes for iOS - you can download it from here. Nevertheless, keep in mind that not every AV software can remove every infection. So if you are unsuccessful with Malwarebytes, you should try other reputable vendors like Bitdefender, Avira, Norton, and many others.
Good luck with the removal
